JOB SUMMARY
This job is responsible for working in a collaborative manner with multi-disciplinary medical center in the continuum of care of these patients, their caregivers and/or families to achieve desired goals. 

The Patient Navigator provides specialized care coordination and navigation support for patients with inherited bleeding disorders within a high-volume Hemophilia Treatment Center. Serves as a central point of coordination for clinic scheduling and patient access, managing provider schedules and appointments using EPIC and manual processes to support efficient clinic operations and continuity of care. Assists patients in navigating insurance coverage, including verification of benefits and addressing coverage-related concerns, and supports coordination of prior authorization processes in collaboration with clinical staff and pharmacy partners. Facilitates timely access to care by coordinating referrals and ensure appropriate follow-up.

EXPERIENCE AND EDUCATION

Required

• Education
  High School Diploma or equivalent. 
   

• Experience
  5 years of working experience in health care environment. 
   

Preferred

• Education
  Bachelor’s Degree in appropriate field.

  Working experience in a large multi-specialty system. 
   

JOB DUTIES

• Acts as a liaison between the patient, their family and internal/external service providers.
• Implements individualized patient navigation plan for patients seeking treatment by facilitation of appointment scheduling, assisting with referral process, providing client education and linking clients with appropriate support services.
• Identifies patient’s understanding of medical care/outcome and provides appropriate education and resources to address needs.
• Empowers patients to self-navigate the healthcare system. Assists people to create their own/unique solutions by acting as a resource of health care information, community support group.
• Advocates teamwork and multidisciplinary care concept to patients, staff and faculty.
• Promotes the navigation program to the UT Southwestern Medical Center (UTSW) community.
• Identifies and maintains a database of appropriate resources for client support.
• Recognizes barriers to healthcare and connects patients to the resources they may need.
• Duties performed may include one or more of the following core functions:
  a) Directly interacting with or caring for patients;
  b) Directly interacting with or caring for human-subjects research participants;
  c) Regularly maintaining, modifying, releasing or similarly affecting patient records (including patient financial records); or
  d) Regularly maintaining, modifying, releasing or similarly affecting human-subjects research records.
• Performs other duties as assigned.
